These Terms and Conditions set out the terms and conditions upon which Roxane Bakker (the
“Practitioner”) is to provide nutritional advice (the “Service”, as further defined below) to you
(“You” or the “Client”) and will apply to the exclusion of all other terms and conditions (other than those which cannot lawfully be limited or excluded including any rights which You are entitled to when dealing as a consumer). Any variation or addition to these Terms and Conditions shall only be valid if made in writing and signed by the Practitioner.
PROVISION OF THE SERVICE
1.1 The Service consists of the provision to You of individual diet related advice and consultations
including the collection and assessment of information relating to Your current diet and the
making of recommendations as to how that could be adjusted to address Your individual goals or concerns. However, as all individuals are different it is not possible to guarantee that any specific result will be obtained as a result of the Service.
1.2 The Service does not include (and the Practitioner shall be under no obligation to provide to
You) any services which it would be unlawful, illegal or unethical to provide or which would require the Practitioner to act in breach of any code of conduct or other regulation to which she is subject.
1.3 Payments can be made by bank transfer. You shall be invoiced for payment after the
consultation and this should be paid within 2 weeks of the consultation.
Price: £75 per hour of nutrition consulting. £37.50 for 30 minutes.
You are required to pay for sessions, which are missed or cancelled, with less than 24 hours’ notice.
It is important that you attend sessions regularly. However, if you do need to cancel an
appointment please let me know with at least 24 hours’ notice by emailing:
1.4 Everything discussed during the nutrition consultations is confidential. If your Practitioner has
serious concerns about your safety or the safety of another person normal confidentiality may be lifted. You will be asked to provide your GP’s contact details, as someone they may contact. The Practitioner keeps a record of your name, contact details and dates and times of appointments on a password-protected database. Your Practitioner is required to keep brief notes about the focus of your work. These are kept securely and are password protected.
The Practitioner requests that the Client notes the following:
• The degree of benefit obtainable from nutritional advice may vary between clients with
similar health problems and following a similar dietary and lifestyle intervention
program.
• Nutritional advice will be tailored to support health conditions and/or health concerns
identified and agreed between both parties.
• Dietitians are not permitted to diagnose, or claim to treat, medical conditions.
• Nutritional advice/dietary and lifestyle is not a substitute for professional medical advice
and/or treatment.
• Standards of professional practice in Nutrition and dietetics are governed by the HCPC
(healthcare professionals council).
• Diet and lifestyle advice is recognised as a complementary method of therapy and is
relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Practitioners consider each individual to be unique
and recommend personalised nutrition and lifestyle programmes rather than a 'one size
fits all’ approach. Practitioners never recommend dietary lifestyle interventions as a
replacement for medical advice and always refer any client with 'red flag' signs or
symptoms to their medical professional.
The Client understands and agrees to the following:
• I am responsible for contacting my GP about any health concerns.
• I give permission for you to contact my GP regarding any agreed aspects of my case.
• If I am receiving treatment from my GP, or any other medical provider, I should tell
him/her about any nutritional strategy provided by my Practitioner. This is necessary
because of any possible reaction between medication and the nutritional program.
• It is important that I tell my Practitioner about any medical diagnosis, medication, herbal
medicine, or food supplements, I am taking as this may affect the nutritional
program.
• Recording consultations using any form of electronic media is not allowed without the
written permission of both me and my Practitioner.
